But a renamed bmp file *will* work, it's just not the best solution.
I am on a Mac, so IrfanView is not an option, and I usually don't want to pay for the favicon.com people to make one for me. I had forgotten that, while I had ruled those options for me a long time ago, other folks might want to use them.
Does anyone know of other ways to make one?

Some graphics programs will save in .ico format. I know that my PhotoImpact (Ulead) does. Oddly, my Adobe Photoshop doesn't, but that may be because it's an older version. Not sure if the Mac versions of PI includes the .ico formatting, but I would assume it does.
It's been awhile since I tinkered on a Mac, however I seem to recall that ResEdit can be configured for icon files. It's available for free from Apple.com, but you'll want to read up on it first to make sure my memory is correct.
There were also a couple of other shareware programs that could handle or convert ico for Mac users. I don't have URL on them anymore but you should be able to find them with a search on the software name. One was called GraphicConverter and the other was called Iconographer.
