You have the primary sitemap index referencing another sitemap index. This is subsequently creating cascade issues in the XML tree as and when it is concatenated into a single document model by the XML processor. It is not particularly a "Google issue", but a limitation of XML processors/processing.

The sitemap index should only reference other sitemaps that detail the URLs on your site, rather than having to trigger an intermediary process to  extract a XML tree from a separate document, you have to reference the final XML document to be included.

Google's documentation on multiple sitemaps probably should make that distinction clear  rather than assume a basic level of XML competence that, with all the best will in the world, will probably not be there with the average website owner, though to be fair, it is heavily implied, particularly if one also reads and understands the sitemap protocol.
