I've got a four year old website, hosted in the USA. My visitors are overwhelmingly foreign, 98% are from outside the USA. Go figure, but it's true. [Nothing from China at all, though. I must be blacklisted by the ghost of Mao.]
I've got good daily traffic numbers, and have monetized the site with Adsense. I have a very good and consistent Adsense CTR, but the average revenue per click has gotten lower and lower over time.
There must be a way for me to address my international audience with better targeted contextual ads. There does not seem to be a way to do that with Adsense. I can create an Adsense Google search box but that's about it.
Another improvement may be to place affiliate program ads on my site. My existing Amazon.com affiliate ads have yielded z-e-r-o because I think nobody in the eastern hemisphere wants to mail order stuff from the USA. There are foreign Amazon.com flavors, German, UK, French, etc., but they seem to be for foreign hosted sites and want foreign tax ID info, etc.
1. Is there a good quality international Adsense-like provider out there? The ones I've browsed seem so scary and scammy.
2. Is there a good quality affiliate program that would target my international visitors better? Ditto - the ones I've seen are rude and crude looking.

The "international" side takes care of itself. Google targets ads at visitors to the site. So if I (based in the UK) viewed your site, I'd probably see different ads than you saw yourself.Your real issue is the declining revenue per click. This could be because your site has been taken out of many advertisers' placement targeting options owing to low conversion rates in the past.
Stop focusing so hard on "International", and start focusing on "Optimization" and "Targeting". Try reading the Optimization Posts from the Adsense blog, this info on section targeting and also through the following recent troubleshooting posts in the Adsense blog:
