I'm in the process of creating an email template to send to my company's customer base to solicit online customer reviews on two sites that currently are only showing old, outdated negative reviews (most recent reviews are from 2005). We're not interested in fabricating any of these reviews, rather we want our happy customers to express their positive experiences on those two sites and possibly turn these negative listings into positive ones.
So, I need some suggestions on how to word this, because I'm getting stuck on what exactly to say in the email.

I would think something simple. Just tell them that you'd like feedback on how they enjoyed working with your company (or its products or whatever) and would they please visit here to provide theirs.
Of course, you'll need to be sure these are happy satisfied customers!

Also, like anything else, I have found that if you offer even a small incentive, you will get better and more responses.
