Anyone know anything about this, or how its done? Or how to even create sites in other languages?

A lot of times I see people get all fired up about wanting to rank well in foreign-language SEs without examining the consequences -- for instance, that people who find them with a website in a particular language will expect them to be able to respond to emails and answer the phone in that language.
As to how to make foreign language websites, as long as you don't give a rat's patootie about the quality of the results, you could always run the existing site pages through any of the free translation tools out there (only cost a few minutes of your time), slap the resulting crap up somewhere online (also very cheap), point a few links at the pages to get them indexed and pat yourself on the back for how you're now ranking in foreign-language SEs for any of the several nonsensical phrases the translation tool mangled your original text into (relatively easy since nobody -- or at least, nobody who actually speaks the language -- is targeting those phrases)...
All told (including placing the links on other sites under your control), you could have the whole thing done in no time and at little to no expense. Of course, you'd also see little to no results from your "work," but, hey, I bet the "optimizers" don't promise anything about your making any more sales anyway.
Or you could do keyword research in the foreign languages (not always easy), hire real live experienced human translators to translate your existing pages (expensive) or foreign-native copywriters to re-do your pages from scratch in the desired languages (mucho expensive), and optimize just as you would for English language pages (takes time and skill).
But since creating one such quality foreign-language website alone would cost waaaaay more than the so-called optimizers charge, my guess is their approach would be much closer to the first option than the second.
It's and should be ignored, IMO.

If you write a page featuring a nonsense English phrase, you can potentially rank in English-language Google for that phrase as soon as the page gets indexed. It's easy to be #1 when you're the only page on the web that features that phrase.
If you create a page with a nonsense phrase in Spanish or any other foreign language, you can likewise probably get it ranked as soon as it's indexed -- because it's likely nobody who actually speaks the language is targeting, or even using, that phrase.
That's how these "foreign language optimizers" get away with it. They count on your not being able to recognize the stupid-sounding nonsense phrases for the crap they are.
Now, if your client wants to rank well for "money" phrases -- those that will actually attract customers who might buy something -- that's likely going to take considerably more optimization than what these folks are offering. The answer in that case is to hire a real SEO who specializes in that language -- not some spammer who sends you an unsolicited email about how you're not ranking in foreign search engines.
(Of course your client doesn't rank in foreign-language search engines! From what you've said, they don't have a foreign-language website, which is something of a prerequisite for foreign-language SERP rankings... )
And, as I alluded to above, if they somehow get rankings in foreign-language search engines that result in real targeted traffic, your client had better be prepared to deal with these customers in their native language. Unless you speak, read and write Spanish (or German or Mandarin or Swahili or whatever) or have someone on your staff who does, why does it even matter how you rank in a search engine in that language?
