About a month ago, we redid our Google ad units to meet their specifications regarding where they could be on the page, where the "Ads by Google" header had to be, etc.  We used Javascript code examples from a Google operative to set it up.  Everything was fine for a while, but in the past few days, we've started to notice that sometimes on a page with multiple ads, only the first ad will be displayed, and the others just show the text "undefined."  Also, when this happens, the one ad that's displayed always seems to be for cars-dot-com.  But then reload the page, and it might show all the ads normally again, and not include that one.

Has anyone else seen this, or know what could be causing it?  The boss is worried that it's some kind of penalty, but she's afraid to even ask Google because every time she talks to them about the ads they come up with something new that must be changed.  I can't imagine a penalty that's intermittent and still allows one particular ad to be displayed, but I don't have a better explanation either.
