Did Link Updates Hurt My Site Rankings?

1 - updated existing links. I re-wrote my link information so that my key terms are in the anchor text (they were not before). I didn't removed any important keywords from the description. Then, I contacted roughly 40 sites that were linking to me and asked them to update my links.
2 - reciprocal linking. I haven't done this that aggressively, but when a site contacts me about this, I have generally obliged. I have done maybe 5 or 7 reciprocal link exchanges in this past 2 weeks.
My ranking on 5 key phrases has dropped somewhere between 5 and 15 spots in the past 3 days. If someone can shed some light on this, that would be great.

That said, be a bit careful in your reciprocal linking practices. Don't link to a site just because they request a link. Make sure the site meets your standards for what you would like your visitors to see. Remember, when you link to someone you are Voting for their site, giving it your stamp of approval. So if you start linking out to a bunch of junk sites it can have a definite negative effect.
Also, as you'll see mentioned around here often you need to target a lot more than 5 keyword phrases. I've yet to see any site that couldn't be targeting at least a couple hundred phrases. And you don't want to end up painting yourself into a corner.
