NoFollow cannot be set by an .htaccess without doing some pretty sophisticated scripting/cloaking. The same is true of robots.txt. Though with robots.txt they can exclude spiders from the pages where your links appear.
It doesn't happen often, but is worthwhile to check whenever you're paying for a link.
